Welcome to Old Brownlee Community Care Center, a skilled nursing and rehabilitation center that ushers in both a new building and a philosophy that honors the individual and their choices.

From the moment visitors walk in the door, they will notice that Old Brownlee Community Care Center is different, but the difference goes much deeper than the physical plant. The difference is the philosophy of person-centered care.

Our center is designed to support this philosophy in a form follows function sort of way.

Old Brownlee features three households, each with a kitchen, dining area, living area and outdoor space designed to place the resident as the center of attention and ensuring they have easy access a variety of spaces. The common area is surrounded by private rooms each with a private bathroom. Households have their own front porch where residents can enjoy outdoor space and visitors may enter through the front porch or main entry.

The resident is the focal point by design and philosophy. Staff are dedicated to individual households and are encouraged to dine with residents and join them in activities such as cooking a favorite dish. This interaction fosters an atmosphere where staff become extended family and really get to know residents, their likes, dislikes and allows them to pick up on any changes more quickly.

Common areas abound from the intimate to the spacious. At the café residents and their guests may help themselves to coffee, cappuccino, beverages and soft serve ice cream – a favorite of the grandchildren!

Staff participate in ongoing culture change education, learning how to honor choice in daily activities such as dining, natural awakening and bath times. We are committed to continuing our journey towards and invite you to learn more about our quest.

Activities and Amenities

Our communities offer a wide array of activities. We offer trips out in the community as well as a variety of life enrichment activities. Amenities range from salon services and fitness centers, to cafes and libraries. Families, community members, and volunteers are invited to join in social activities.

Who We Are

Established in 1994, CommCare Corporation is a Louisana-based non-profit that began as owners and operators of skilled nursing and rehabilitation centers. CommCare has since embarked on a journey to provide more integrated, person-centered transitional care throughout the care continuum.

Rehab to Home

While many residents are able to return home, our goal is to get each individual functioning at their highest level and capability through physical therapy, occupational therapy and speech therapy. Our fully equipped rehabilitation gym is staffed by physical therapists, occupational therapists and speech therapists.

Volunteers

Volunteers are integral to the CommCare experience, and volunteering at a CommCare community or hospice can be a rewarding experience! Teach an instrument, voice, or dance? Like to cook? Love to travel? We would love for you to share your passions with our residents and patients. The possibilities are limitless!

Resources

As a non-profit organization, part of our mission is education and outreach. We welcome you to use the resources in our library to get information on topics facing elders and their families. Resources in our library are targeted toward elders, and range from topics that address health and wellness, medical conditions, as well as information concerning Medicare or Medicaid benefits.

A: No. It is licensed by the State of Louisiana as a skilled nursing and rehabilitation center. Old Brownlee Community Care Center offers long term care as well as rehabilitation necessary for individuals to regain strength, endurance and skills to return home successfully.

A: Old Brownlee accepts Medicaid, Medicare, some insurance as well as private payment.

A: Old Brownlee Community Care Center borrows its name from the Old Brownlee Road where it is located. Early in the 19th century Brownlee was a flag station on the Cotton Belt Railroad about five miles out of Shreveport. Mr. W. Boney had a general business there. Brownlee Road and Brownlee Estates harken back to that flag station just north of Bossier City.

A: Old Brownlee offers physical therapy, occupational therapy and speech therapy that help individuals gain function and ability after an illness, injury or condition. While Old Brownlee provides long term care, many people are able to complete rehabilitation and return home successfully.

A: Old Brownlee offers only skilled nursing and rehabilitation services.
